To streamline your first visit, please print and complete the patient documents listed below and bring them with you to your appointment. If you prefer, you can fill them out in our office, but please allow extra time before your appointment.

We gladly accept the health insurance plans listed below*. If your plan is not listed, please call us at (773) 609-1847 as we are always adding new insurance plans.

As a courtesy to our patients, we will verify your health benefits prior to your arrival to determine if there will be a copay, deductible, or coinsurance for therapy services.

For those patients who do not have health insurance or those who have maxed out their benefits, PREP Performance Center offers a $140.00 per visit, self-pay fee. Please contact us if you have additional questions.

To help ensure a smooth and productive first visit, here are a few friendly reminders and tips:

Arrive 15 minutes early for your first appointment to complete any necessary paperwork.
Bring a copy of your insurance card and a photo ID with you.
